Madison Palmiero MS, OTR/L, CHT
Certified Hand TherapistMadison was born and raised in Louisville, Kentucky. She graduated Cum Laude from the University of Louisville with a Bachelor of Science degree in Health and Human Performance with a concentration in Exercise Science. She graduated from Spalding University with her Master of Science degree in Occupational Therapy in August of 2018. Madison has practiced Occupational Therapy at Commonwealth Hand Therapy, Since October 2018. She earned the title of Certified Hand Therapist in December of 2021.
Madison is committed to collaborating with clients to reach their full potential through activity analysis, creating custom-made orthotics to improve function or correct issues impairing function such as muscle and tissue tightness or joint stiffness.
Madison has a special clinical interest in treating distal radius fractures because she enjoys the problem solving required to return function. Outside of the clinic Madison enjoys spending time with her Siberian Huskies, fishing, kayaking, lifting weights, and boxing.

Occupational TherapistAdam is an occupational therapist originally from Simpsonville, Kentucky. After obtaining his Bachelor of Science in Biology from the University of Kentucky, he secured a spot in the inaugural class of the entry-level Doctor of Occupational Therapy program at Eastern Kentucky University in 2020. After completing the program, Adam began working at Commonwealth Hand & Physical Therapy, where he completed clinical fieldwork during OT school. Now, Adam is working toward becoming a Certified Hand Therapist. Outside the clinic, Adam loves roaming, hiking, reading books, and watching good movies.

Certified Hand TherapistRyan is a 2002 Occupational Therapy graduate from Eastern Kentucky University. He has worked primarily in the arena of hand and upper extremity rehabilitation with traumatic and cumulative trauma cases for 16 years and a Certified Hand Therapist (CHT) for 11 years.
Ryan has focused particularly in industrial rehabilitative services and is currently the lead hand therapist on site at Toyota Motor Manufacturing Kentucky (TMMK) acute care clinic, with clinical focus and training in the realm of Functional Capacity Evaluations (FCEs), ergonomic evaluations, post offer/pre-employment testing, job analyses and preventative programs.

Occupational TherapistDaniel is originally from Lexington, Kentucky, and recently moved back from New Mexico with his wife. He graduated from the University of Kentucky in 2020 with a Bachelor’s degree in Exercise Science and later earned his Master of Science in Occupational Therapy from Milligan University in Johnson City, Tennessee, in 2023. Daniel has always had a strong interest in hand therapy and is excited to help others in their recovery while growing under the gifted mentorship of the Commonwealth team. He aspires to become a Certified Hand Therapist. Outside of work, he enjoys spending time with friends and family, as well as his cat, hiking, playing pickleball, and playing the drums.
Chad Hamilton PT, DPT, COMT

Chad is a Physical Therapist and Certified Orthopedic Manual Therapist from Owensboro, KY. He graduated from the University of Kentucky with a degree in Exercise Science in 2010 and received his Doctorate of Physical Therapy from the University of Kentucky in 2014. He has been practicing in outpatient orthopedics for over 5 years and his clinical interests include injuries of the spine, shoulder, and upper extremity injuries, lower extremities, vestibular dysfunction, TMJ dysfunction, and dry needling. Chad obtained his Certification in Orthopedic Manual Therapy (COMT) in 2019 from the Institute of Advanced Musculoskeletal Treatments (IAMT). Outside of the clinic he enjoys hiking, traveling, watching sports, playing with his dog Leo, and spending time with his family and friends.

Physical TherapistStephanie is a physical therapist originally from Cleveland, OH. She earned her Doctorate of Physical Therapy in 2022 from the University of Kentucky, where she also received her Bachelor of Science in Kinesiology in 2017. Before joining Commonwealth Hand and Physical Therapy, Stephanie has worked in various settings, including acute care, inpatient rehabilitation, and at an orthopedic clinic working with professional and collegiate athletes.
Outside of the clinic, she enjoys watching Formula 1, Kentucky Basketball, Lexington Soccer Club, traveling, and spending time with her husband, daughter, and dogs.
